The We Care Card initiative is designed not only to support health and wellness in the community, but its purpose is also to educate DuPage Township residents about the benefits of good nutrition and fitness, plus the need for consistent management of health issues. DuPage Township has invested $20,000 to bring this program to township residents. Township and hospital officials expect that this program’s return on that investment will be that it will help reduce the number of emergency room visits for non-emergency health care.
The 10,000 families and 2000 seniors served by the DuPage Township Food Pantry are receiving notification about the We Care Card initiative; however, any township resident may apply. Click here for the application.

- What is an initial personalized health assessment?It is a confidential questionnaire used to evaluate your health profile and risk factors. It will be reviewed with you and one of the program coordinators to determine next steps.
- If I am eligible for benefits, how does the process occur?After the initial screening is complete, you will be contacted by a member of the team to set up an appointment. During that appointment we will determine what additional lab work or tests need to be admininstered as well as develop any treatment recommendations.
- What if I already have a personal physician?At your request, we will work directly with your physician to determine treatment recommendatons.
- What if I do not have a personal physician?The staff at Adventist Bolingbrook Hospital would be glad to help you find a physician that meets your needs.
